Interoceptive Awareness and Eating

How sensing internal body states connects to eating behavior.

What the evidence says

  • Interoception is the perception of internal body states (hunger, fullness, emotion)[1]
  • Interoceptive awareness is multidimensional — the MAIA measures 8 distinct scales: noticing sensations, not distracting from them, not worrying about them, regulating attention to the body, emotional awareness, self-regulation, body listening, and trusting the body[2]
  • Mindfulness training improves interoceptive awareness — but its effect on healthy eating adherence *did not reach significance across the whole sample*[1]
  • 2023 RCT (Loucks, JAMA Network Open): the MB-BP program raised MAIA interoceptive-awareness scores by 0.54 points at 6 months (95% CI, 0.35-0.74; P < .001; Cohen d = 0.45)[1]

  • *Three interoceptive-awareness items bridge to eating-disorder symptoms and the paper says they do not differ. The highest bridge EI (.20) belongs to "feeling that one's body is a safe place", which "had significantly higher bridge EI than 91% of other bridge nodes"; "(mis)trust in body sensations" was higher than 47% of nodes and "ignoring physical tension/discomfort" than 42%. The authors state that "these three top IA bridge symptoms were not significantly different from each other (ps > .05)"*[4]
  • *People better at detecting their own heartbeat show larger neural responses to emotional pictures. In an EEG study of 37 people — 17 classified good and 20 poor heartbeat perceivers on Schandry's counting task — good perceivers had significantly larger P300 and slow-wave amplitudes viewing emotional images, and heartbeat-perception scores correlated with both those amplitudes and subjective arousal ratings. Body awareness and emotional awareness are linked at the neural level, in a small cross-sectional sample*[5]
  • The body scan trains attention to internal signals; the framing that bodily awareness carries knowledge cognitive analysis misses is Kabat-Zinn's own, from a book this library holds the existence of rather than the text[6]
