A landmark reframing of the First Law of Thermodynamics as applied to obesity: rather than overeating and inactivity causing weight gain, these behaviors are secondary consequences of obligate weight gain driven by hormonal dysfunction. Uniquely integrates three mechanisms — leptin resistance, hyperinsulinemia as a primary driver, and insulin-leptin signaling crosstalk — into a single model. Also identifies hyperinsulinemia's interference with dopamine clearance in reward centers as a pathway to increased food reward.
