A reframing of the First Law of Thermodynamics as applied to obesity: rather than overeating and inactivity causing weight gain, these behaviors are secondary consequences of obligate weight gain driven by hormonal dysfunction. Its abstract, in Europe PMC’s text, names its own three mechanisms: weight gain ”is driven by the hyperinsulinemic state, through three mechanisms: energy partitioning into adipose tissue; interference with leptin signal transduction; and interference with extinction of the hedonic response to food” (read 2026-09-10; the held abstract file lacks this sentence). (Corrected 2026-09-10 from a triad the record supplied, ”leptin resistance, hyperinsulinemia as a primary driver, and insulin-leptin signaling crosstalk”, which the 2026-09-02 note below called the checkable fact.) Also identifies hyperinsulinemia’s interference with dopamine clearance in reward centers as a pathway to increased food reward. Supported by Lustig’s own 2004 octreotide trial, of 17 subjects, which the note held here says ”suggests direct relationship between reduced insulin and improved leptin sensitivity” (corrected 2026-09-10 from ”demonstrating that pharmacological insulin suppression ameliorates leptin resistance”; the study is not held, and its figures are the 6 responders’).
